Terms of use

These terms apply to everyone who uses the MCR Health Direct Primary Care enrollment app: employers, their employees, and MCR Health staff.

Not insurance, not medical advice

Direct Primary Care is a sponsored membership for everyday primary care. It is not health insurance and does not replace major medical, hospital, or specialty coverage. This app provides enrollment and member card functions only; it does not provide medical advice and is not for emergencies. In an emergency, call 911.

Accounts

  • Employers are responsible for the accuracy of their roster and for removing staff who leave.
  • Accounts are individual. Passwords must be at least 12 characters and must not be shared. You are responsible for activity under your account.
  • Employees do not self-enroll; an employer administrator creates the account and MCR Health issues the Patient ID.

Member cards

A digital member card shows current enrollment at the moment it is displayed. It is not a payment card and confers no benefit on its own. Eligibility is confirmed by MCR Health at the time of service, and a card for a disabled or removed employee is no longer valid.

Acceptable use

Do not attempt to access another company’s information, another person’s card or account, or any part of the app beyond your permission level. Do not submit false enrollment information. Access may be suspended for misuse.

Plans, changes, and cancellation

Plan terms, services, and pricing are governed by the contract between the employer and MCR Health. Requests submitted in this app — plan changes, roster changes, or cancellation — take effect only once MCR Health confirms them.

Availability and changes to these terms

The app is provided as-is and may be unavailable during maintenance. MCR Health may update these terms; continued use after an update means you accept the revised terms.
