Privacy notice

This notice explains how the MCR Health Direct Primary Care enrollment app handles information about employers and their employees. It covers this app only; care delivered at an MCR Health practice is also governed by MCR Health’s clinical privacy practices.

Information we collect

  • Employer account details: business name, contacts, plan selection, and the actions administrators take in the app.
  • Employee enrollment details: name, date of birth, work email, assigned plan, status, and the Patient ID issued by MCR Health.
  • Messages and requests: the change requests and messages exchanged between an employer and MCR Health staff.
  • Activity records: a log of account, roster, and plan changes, including who made each change and when.

How information is used

Information is used to enroll a business, keep its roster accurate, issue and display member cards, verify eligibility at check-in, respond to employer requests, and maintain an auditable record of changes. It is not sold, and it is not used for advertising.

Who can see what

  • Employees see only their own member card and account details.
  • Employer administrators see their own company’s roster and plan, with employee names shown in abbreviated form. They cannot see other companies.
  • MCR Health staff see the information their permission level allows, which may include full rosters for eligibility verification and plan administration.

Clinical records

This app does not contain medical records, visit notes, diagnoses, or treatment history. Clinical information stays in MCR Health’s medical record systems and is protected under applicable health privacy law.

Security

Access requires an individual account with a password of at least 12 characters. Access is restricted by role, changes are logged, and data is stored in an access-controlled database.

Retention and your choices

Enrollment and activity records are retained while a company’s contract is active and for as long as needed afterward for recordkeeping obligations. Employees should contact their employer administrator or MCR Health to correct their details.
